I use the standard mandatory Louisiana purchase agreement, note in remarks that it is a lease purchase, and then accompany it with a comprehensive Buyer to Occupy Property Prior to Act of Sale Addendum detailing how much the rent is, how much (if any) will apply toward purchase, duties of buyer in maintaining property & insurance on contents, duties of seller in paying taxes & insurance, etc.
