How do I recieve help in refinance of my home if I to this point never missed a payment

We are retired and have a medical hardship.  We continue to make our payments on time even though it takes almost most of our income to do so.  When we have tried to refinance we have been told that we owe the same as our house is worth now due to the economy.  What happens to people who try to do the best and now that we need help there is no help  out there.
