Typically you'd sign a quit-claim deed and record it with your town clerk.  This may be a technically violation of your mortgage terms (assuming you have a mortgage) because you'd be selling (effectively) half the property to your wife.  That being said, banks rarely care and rarely find out.

An attorney can prepare the proper paperwork... it's usually around $200 including the recording fees.
