the 2nd does not have to stay in place; a person is free to pay if off by other means (i.e. cash).

Yes, but the second mortgage holder will have to agree to subordinate to the new first mortgage.

................and not all lenders will agree to subordinate. check with the 2nd mortgage lender to see if they will before proceeding with the 1st mortgage.

Yes, you can only refinance the 1st mortgage NOT the 2nd mortgage.  Need to subordinate only.  ps...Every lender has its own rules for subordination.
