Here's a link to an excellent blog on doing FSBO, with some recommendations on dealing with some of the contractual aspects.

I'm always amused at the comparison of real estate with an MD.  It takes less than 12 weeks to get a real estate license after high school, vs 12 years or more for one to have credentials as a surgeon.

Sounds like you need to contact a lawyer and have someone give you guidance on how to sell your house. It is not as easy as filling in the blanks.

Since you appear to be unfamiliar with the process, I suggest you interview 3-4 local Real Estate agents and place your house on the market through one of them.  It will save you money and time in the long run. 

Do you really want to sell a major asset without proper guidance?
Kind of like performing surgery on yourself, sure you can do it, but probably not the best idea.
