You may want to consider using a Realtor, one who would look after your best interest. He or she will be familiar with the proper forms that are required. Laws change quickly and you want to be protected.

There are FSBO contracts out there but you really should not need to use them. If the purchaser is going to have a mortgage, an attorney is going to be involved in the process anyway and should be able to get you a contract.
