Each lender has it's very own loan modification department.  If you look at your mortgage statement it should have a number to call.  Call that number and ask  to speak to their modification department.  They should ask you some questions and I am almost positive they'll have to mail you a package with forms to fill out.  You'll have to fill out the forms and supply the requested documentation.  I also think it would be a good idea for you to ask them if they have a local help center where you could go in and sit down and talk to a live person.  I am sorry for your hardship and wish you the best - the process may take some time but be assertive and don't give up.
